Cloud AI · The Tier 1 Approach

Enterprise cloud AI platforms now offer BAA-covered tiers that make routine bureaucratic work in solo dermatology automatable: pathology reports, referral letters, insurance forms, patient communication, knowledge surveillance, and visit documentation. The "local extraction, BAA-covered analysis, structured output" pattern is generalizable across all of these. PathCorrelate is one illustrative implementation focused on automated pathology correlation.

Background & Methods

Solo dermatologists deliver most outpatient care yet are largely excluded from clinical AI. The administrative and documentation burden (pathology reports, referrals, insurance forms, patient communication) consumes 20-35 min/day, ~140 hr/year per practice, a leading driver of burnout.

Narrative review through May 2026 of BAA-covered enterprise AI applicable to common dermatology workflows; vendor documentation reviewed for Google, Microsoft, AWS, Anthropic, OpenAI, xAI.

Generalizable Four-Step Pattern

  • Extract locally: text or fields via PyMuPDF/local OCR; zero PHI egress
  • Store in BAA-covered cloud (Drive, OneDrive, S3 with appropriate BAA)
  • Analyze with BAA-covered AI (Gemini 3, GPT, Claude via Bedrock) for structured fields
  • Output to a structured target: Sheets, EHR field, letter template, patient SMS, action plan

Conclusions

BAA-covered enterprise cloud AI is a practical substrate for automating routine document-heavy workflows in solo dermatology, compliant with Amendment 13, at a price point accessible to solo practice.

The "local extraction, BAA-covered analysis, structured output" pattern is generalizable well beyond pathology: referrals, insurance, patient communication, and continuous knowledge surveillance.

Added value: the barrier to adoption is informational, not technological. No custom server, no commercial middleware, and minimal technical expertise required.
